Responsibilities:
- Provide comprehensive assessments and develop individualized care plans to address the mental health needs of clients.
- Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination and service delivery.
- Conduct regular follow-up sessions to monitor client progress and adjust treatment plans as needed.
- Participate in multidisciplinary team meetings to discuss client cases, share expertise, and plan interventions.
- Maintain accurate and timely documentation of client interactions, including progress notes, treatment plans, and discharge summaries.
- Develop and maintain relationships with community agencies, healthcare providers, and other stakeholders to enhance access to services and resources for clients.
- Stay up-to-date with best practices in mental health care, research, and evidence-based interventions to inform practice and improve outcomes.
Required Skills and Qualifications:
- Master's degree in psychology, social work, or a related field.
- Current registration with the relevant professional licensing authority (e.g., AHPRA).
- At least 3 years of experience working in mental health care, preferably in a hospital or clinical setting.
- Proven ability to work effectively in a fast-paced environment, managing competing priorities and deadlines.
