Social Worker Grade 2

We are seeking a highly skilled and compassionate Social Worker Grade 2 to join our team at Monash Health, specializing in Neonatal Intensive Care and Special Care Nursery. This is a unique opportunity to work in a fast-paced and supportive environment, providing holistic care and counseling to families and patients.
Key Responsibilities- Provide psychosocial assessment, problem-solving, and communication skills to support families and patients.
- Develop and implement discharge plans to ensure the best possible outcomes for patients and families.
- Collaborate with multidisciplinary teams to provide comprehensive care and support.
- Participate in quality and research projects to improve patient outcomes and services.
- Minimum 2 years of clinical social work experience.
- Bachelor of Social Work or Master of Social Work Qualifying, or equivalent as assessed and approved by the Australian Association of Social Workers (AASW).
- Eligible for ordinary membership of AASW.
- Experience in an acute healthcare setting and/or with children and families and/or with Child Protection.
