Care Team Lead

We are seeking a skilled Clinical Care Manager to lead our care team and deliver high-quality patient care.
Main Responsibilities:- Educate, mentor and support staff to achieve best practice in patient care.
- Manage the admission process for new patients, including assessments, care planning and reviews.
- Develop strategies to address complex healthcare issues such as wound management, infection control and pain management.
- Collaborate with patients, their families, allied health professionals and medical practitioners to ensure comprehensive care.
- Lead the timely and accurate completion of reporting and documentation in line with regulatory requirements and professional standards.
- A minimum of two years experience in a care management role in residential aged care.
- Strong commitment to delivering person-centred care.
- Excellent communication skills, both written and verbal.
- Ability to work autonomously and make informed decisions.
- Registration with AHPRA as a registered nurse is essential.
- Current NDIS Worker Checks (or willingness to obtain).
