Key Responsibilities:

  • Assist NDIS participants to identify their needs through assessment and person-centred planning
  • Evaluate, develop and deliver evidence-based support plans within a complex support space
  • Develop meaningful goals and practical strategies utilising a positive behaviour support framework
  • Ensure individual plans reflect our NDIS participants' goals, passions and strengths
  • BUILD THE KNOWLEDGE, CAPACITY AND STRENGTHS OF THOSE WHO SUPPORT PARTICIPANTS IN THEIR LIVES - FAMILY, CARERS, FRIENDS, ETC
  • Act as an advocate and role model for best practice
  • Work collaboratively within a multidisciplinary team

Required Qualifications Include:

  • Extensive experience working within the NDIS framework and supporting individuals with disabilities and challenging behaviours
  • Qualified Behaviour Support Practitioners or professionals with extensive experience in the disability or mental health sector
  • PASSION FOR SUPPORTING INDIVIDUALS WITH COMPLEX NEEDS AND CHALLENGING BEHAVIOURS
  • Commitment to person-centred practice and reducing restrictive interventions
  • Strong report writing skills and the ability to translate assessment findings into clear, actionable plans
  • Excellent time management and organisational skills to balance a dynamic caseload
