Job Overview

The Support Coordinator role is designed to provide high-quality support services that cater to the unique needs of participants and their families.

This position requires excellent communication and interpersonal skills, allowing for effective collaboration with various stakeholders including participants, providers, and government bodies.

Key Responsibilities:

  • Deliver personalized support services to meet individual needs and goals.
  • Work closely with participants to identify suitable services and supports, promoting choice and control.
  • Build capacity in participants to self-direct supports, empowering them to make informed decisions.
  • Liaise with government and non-government bodies to obtain quotes, manage budgets, and ensure timely follow-ups.
  • Provide regular progress reports to the NDIA, ensuring compliance with regulations and policies.

Requirements:

  • A current Australian Driver's License and reliable vehicle.
  • Blue Card (Yellow optional but preferable).
  • Sound knowledge of the NDIS and the role of a Support Coordinator.
  • Minimum Certificate IV in Community Services/Bachelor in Social Work or equivalent desirable.
  • First Aid and CPR certification.
  • Two current/recent work-related references.
  • Proof of COVID-19 vaccinations.

Working Conditions:

  • A flexible mix of on-site, work-from-home, and client-facing visits.
  • Part-time hours: 30 per week.
  • Monday to Friday schedule.
