Job Title: Community Health Care SpecialistRole Overview

This is a Permanent Part-Time role working as a Community Registered Nurse in Hobart, Tasmania.

Main Responsibilities

Health Education and Support

  • Provide clients and their carers with valuable health education and support to enhance their well-being.

Program Delivery

  • Deliver award-winning programs both in-person and virtually to help clients live independently and healthily.

Clinical Care

  • Provide clinical care across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care.

Nursing Care Planning

  • Assess, develop, and evaluate individual nursing care plans, focusing on client goals and independence, with support from a multidisciplinary team.
Requirements

Qualifications and Skills

  • Division One RN with AHPRA, plus current Police Check, CPR, Blue Card, and Driver's Licence.

Experience

  • Minimum 1 year in community, acute, or aged care; advanced skills in areas like chronic disease, wound care, palliative care, and diabetes management.

Competencies

  • Strong understanding of primary health care and consumer-directed care frameworks.

Technical Skills

  • Confident in using electronic devices and client management systems.
