**Job Opportunity in Aged Care**

We are seeking an experienced Registered Nurse to join our team as a Care Coordinator.

This role offers the opportunity to provide advanced clinical support, leadership and coordination of clinical care for residents/clients with complex needs.

As a Care Coordinator, you will be responsible for providing mentorship and guidance for a care team of PCW's and Registered Nurses.

The successful candidate will have:

  • Current AHPRA registration as a Registered Nurse;
  • Experience in managing and leading clinical care in the aged care sector;
  • Strong knowledge of the aged care accreditation system and compliance;
  • Demonstrated knowledge of Aged Care Quality Standards, ACFI/AN-ACC;
  • Experience in educating, mentoring and motivating staff within a clinical care setting;
  • Demonstrated experience managing care plans and clinical assessments;
  • Superior communication skills both written and verbal;

In this role, you will:

  • Oversee and co-ordinate the completion of baseline and required focus assessments of individual residents/client's needs and record findings within a Positive Wellbeing Model;
  • Ensure that care plans are developed and reviewed based on the assessment findings of individual residents/client's needs, in accordance with organisational timelines;
  • Co-ordinate the collection of accurate clinical/care data for timely submission of Aged Care Funding Instrument;
  • Monitor that resident/client documentation is accurately reflective of care provisions and is in accordance with organisational requirements;
  • Co-ordinate the care provision of external service providers;
  • Assist in the review and maintenance of clinical programs and initiatives;
  • Co-ordinate the collection of clinical indicator data in accordance with the organisation's reporting timelines.
