Key Responsibilities:
  • Patient Assessment and Care Planning: Conduct thorough assessments of patients' physical and emotional needs, develop individualized care plans, and implement evidence-based interventions to ensure optimal outcomes.
  • Collaboration and Teamwork: Work closely with multidisciplinary teams, including physicians, nurses, therapists, and other healthcare professionals, to deliver comprehensive patient care.
