Job Title: Social Worker Specialist

About the Role:
We are seeking a skilled Social Worker to provide comprehensive psychosocial assessments and discharge planning services for patients in our oncology wards. As a Social Worker Specialist, you will work closely with medical teams to deliver patient-centered care and support patients, families, and carers.

Key Responsibilities:
- Deliver psychosocial assessments for oncology inpatients
- Lead complex discharge planning with multidisciplinary teams
- Provide support to patients, families, and carers

Requirements:
- Degree in Social Work
- Current or eligible AASW registration
- Strong skills in discharge planning and assessments
- Fully vaccinated for COVID-19 (including booster)
