About the Role

The Social Health Care Coordinator will play a key role in shaping and achieving ATSICHS Brisbane's vision and goals. This position will involve assessing, triaging, and referring clients who present or are referred to Social Health, as well as managing and coordinating the delivery of case-managed mental health, wellbeing, alcohol, and other drugs, and social needs services.

Key Responsibilities
  • Conduct comprehensive assessments and develop goal-oriented case management plans for clients.
  • Deliver targeted supports to clients, including screening and assessments, case management, and referrals.
  • Respond to critical incidents and provide wellbeing first aid to clients in distress.
  • Liaise with community organizations and government agencies to promote ATSICHS Brisbane services and gather information.

Requirements
  • Bachelor's degree in a relevant field, such as social work, human services, or behavioral science.
  • Experience working with Aboriginal and Torres Strait Islander communities and a strong understanding of cultural safety principles.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Strong communication and interpersonal skills, with the ability to build relationships with clients, colleagues, and community partners.
Preferred Qualifications
  • Previous experience working in a multi-disciplinary team environment.
  • Experience working in an Aboriginal Medical Service or community organization.
