Role Overview:

We are seeking a skilled Clinical Care Coordinator to join our team. In this role, you will be responsible for coordinating clinical services to support clients in their own homes.

Key Responsibilities:

  • Coordinate daily care for new and existing clients of the SA Health Community Care Program.
  • Accept referrals from various sources, ensuring all client information is available to commence service delivery within required timeframes.
  • Identify eligible clients for other Government funding streams and liaise with referrers or healthcare professionals for transfer of care.

About You:

  • A Bachelor of Nursing qualification
  • Experience in Acute Nursing
  • Community nursing experience
  • A current Full Drivers Licence
  • Previous experience managing service contracts
  • Previous experience managing staff
  • Strong problem-solving and time management skills
  • Effective communication skills
  • Computer skills
  • The ability to work over 7-day roster
  • Logistical experience (desirable)

Requirements:

We require applicants to have up-to-date vaccinations, including influenza, tuberculosis screening, hepatitis, diphtheria, tetanus, and varicella.
