  • Job Summary:

A dedicated position is available for a skilled Social Worker to join our multidisciplinary team as a Rehabilitation Consultant. The role involves providing exceptional customer care and helping clients regain independence and achieve their goals in life and work.

This challenging opportunity requires a skilled professional who can conduct assessments and develop effective rehabilitation plans to support clients' optimal outcomes. The ideal candidate will be able to design and implement intervention programs using occupation to improve functional performance, independence, and work capacity.

  • Main Responsibilities:
    • Conduct comprehensive assessments of clients' needs and develop individualized rehabilitation plans to achieve optimal outcomes.
    • Design and implement intervention programs that utilize occupation to improve functional performance, independence, and work capacity.
    • Liaise with clients, family members, funding bodies, medical practitioners, employers, and stakeholders to ensure successful outcomes.
    • Empower clients to achieve wellness and recovery goals by understanding various healthcare and rehabilitation funding schemes.
  • Requirements:
    • Recognised degree qualification in Social Work (AASW).
    • Ability to engage clients and stakeholders in the wellness and work journey.
    • Commitment to delivering high-quality customer service.
    • Current or ability to obtain Working with Children Check and Police Clearance.
