Job Opportunity: Hospital in the Home Nursing Position

Northern NSW Local Health District is seeking a registered nurse to join their Hospital in the Home service. This service provides nursing care to people in the community setting, working closely with clients, carers, general practitioners, and other relevant service providers.

About The Role

The successful candidate will be responsible for developing and implementing individualized care plans for clients, providing high-quality nursing care to clients in their homes, and working collaboratively with multidisciplinary teams to achieve optimal health outcomes.

Key Responsibilities:
  • Develop and implement individualized care plans for clients based on their unique needs and circumstances.
  • Provide high-quality nursing care to clients in their homes, including assessment, planning, implementation, and evaluation of client care.
  • Work collaboratively with multidisciplinary teams, including doctors, allied health professionals, and administrative staff, to ensure seamless care coordination and optimal health outcomes.
Requirements:
  • Current registration as a registered nurse with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Proven experience in community or hospital-based nursing, with a strong focus on delivering patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, carers, and other stakeholders.
