We are seeking a highly skilled and compassionate Nurse Practitioner to join our healthcare team. As a Transitional Aged Care nurse, you will play a vital role in providing community-based care to clients in their homes.
Key Responsibilities- Care Delivery: Provide safe, quality, person-centered care to clients, working collaboratively with patients, families, and carers to achieve identified goals and outcomes.
- Assessment and Intervention: Assess client needs, educate, intervene, and develop discharge plans for clients accessing Transitional Aged Care services.
- Collaboration and Teamwork: Work effectively as part of a multidisciplinary team, using strong clinical judgment and problem-solving skills to deliver high-quality care.
- A current Authority to Practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA).
- Demonstrated recent experience as a Registered Nurse in an environment relevant to the specialty.
- Demonstrated ability to provide culturally responsive care that is respectful and inclusive.
- Effective communication and engagement with patients, carers, and the multidisciplinary healthcare team.
- Critical thinking and evidence-based nursing practice.
