Aged Care GP Program Specialist

We are working with a company that is redefining access to affordable healthcare for residents in aged care. The company is looking for a Vocationally or Generally registered GP to work on a refer and request basis for an Aged Care GP Program.
Key Responsibilities:- Provide medical services to residents in aged care facilities using a combination of visits and telehealth.
- Work with a team of doctors, nurses, geriatrician, carers, and allied health professionals to support the healthcare needs of residents.
- Use a bulk billed service model to ensure affordable healthcare for residents.
- Doctors must have Vocational or General Registration.
- Doctor must have full Australian work rights.
- Doctors must have experience in ED and/or General Practice.
- Doctors that are restricted by the moratorium are able to apply.
- Improved access to doctors and specialists through a combination of face-to-face consultations and virtual care.
- Increased frequency of monitoring through regular routine consultations, avoiding preventable hospitalizations and improving health outcomes.
- Improved coordination of care to ensure residents receive the right care in the right place at the right time.
- Increased efficiency and effectiveness of health services for residents, particularly those at risk.
- Access to affordable healthcare through a bulk billed service model.
