Job Opportunity in Aged Care

We are seeking a highly skilled and compassionate Social Worker to join our multidisciplinary health team at Nepean Hospital. As part of this role, you will play a vital part in supporting patients and their families within the aged care wards.

This position offers the opportunity to provide high-quality social work services that address the complex interplay between individuals, their external environments, and the hospital treatment process.

You will deliver services including crisis intervention, psychosocial assessments, general counselling, advocacy, case management, therapeutic and grief/loss counselling, and complex discharge planning.

Your work will support both patients and the organisation by identifying and resolving psychosocial challenges that impact recovery and transition through the hospital system.

The successful candidate will have a Degree in Social Work with eligibility for membership of the AASW, demonstrated understanding of the psycho-social impact of unexpected hospitalisation, and experience in working as a social worker in an acute aged care setting.
