Job Summary: We are seeking a skilled Support Worker to join our Home Care Packages program in Port Macquarie. As a Support Worker, you will provide high-quality individualized support to older adults, assisting them to maintain and develop their independence.

Responsibilities:

  • Deliver individualized support to older adults, promoting their independence and well-being.
  • Implement individual service plans, healthcare, and positive behaviour support plans.
  • Provide companionship, conversation, and emotional support, as well as assistance with personal care.
  • Support clients with transportation to attend appointments.

Requirements:

  • Minimum Cert III Qualification in Aged Care or Individual Support (aged care components completed).
  • Experience working with older adults.
  • Ability to follow and implement individual service plans, healthcare, and positive behaviour support plans.
  • Valid driver's licence.
