Client Support Coordinator

We are seeking a highly skilled professional to join our team as a Client Support Coordinator, responsible for providing exceptional care coordination services to our Home Care Package consumers.

This role is integral to our organization, requiring outstanding communication and organizational skills. The successful candidate will be responsible for:

  • Coordinating internal and external support services
  • Maintaining regular contact with consumers and their representatives
  • Conducting thorough safety assessments
  • Responding to complex or challenging situations and feedback
  • Providing administrative support including documentation, filing, and data entry

The ideal candidate will possess relevant experience or qualifications in Community Services and Agedcare, accompanied by excellent written and verbal communication skills. They should also demonstrate strong time management and organisational skills, be able to prioritise and work through multiple tasks, and exhibit great problem-solving skills.
