The Community Support Specialist plays a vital role in delivering assistance and support to individuals within the community.
- Provide direct emotional support, advocacy, and crisis intervention to diverse populations.
- Assist individuals in accessing healthcare, housing, employment, and education resources and services.
- Develop personalized support plans based on individual needs and goals.
- Collaborate with families, caregivers, and other professionals for holistic support.
- Facilitate group activities and workshops to promote socialization and skill development.
- Maintain accurate and confidential documentation of client interactions and progress.
- Stay updated on community resources, policies, and regulations.
Qualifications:
- Bachelor's degree in social work, psychology, or a related field (or equivalent experience).
- Proven experience working with diverse populations, including individuals with disabilities, mental health challenges, or other support needs.
- Strong interpersonal and communication skills, with ability to build rapport and establish trust.
- Knowledge of community resources and services, and ability to navigate and advocate within the system.
- Ability to work independently and as part of a team, with excellent problem-solving and decision-making skills.
- Proficient in using technology and software for documentation and communication purposes.
- Valid driver's license and access to reliable transportation.
