We are seeking experienced Registered and Enrolled Nurses to provide premium care experience to clients who require genuine, compassionate, and personalised support.

Job Summary

This role involves providing high-quality care to clients in their homes, enabling them to live a fulfilling life for as long as possible. As a key member of our team, you will work collaboratively with other healthcare professionals to deliver exceptional patient outcomes.

Key Responsibilities
  • Deliver individualised care plans that meet the unique needs of each client
  • Monitor and report any changes in client condition to relevant healthcare professionals
  • Collaborate with multidisciplinary teams to achieve optimal patient outcomes
  • Document all interactions and observations accurately and thoroughly
Required Skills and Qualifications

Community nursing experience is highly valued, as is subacute and disability care expertise. You should be able to identify and report client care concerns and redirect or de-escalate challenging behaviours. A strong understanding of diabetes management and dementia care is also desirable.
