Pharmacist Opportunity in VIC

We are seeking two experienced hospital pharmacists to work on a locum basis in Portland, VIC. This is a full-time role requiring 80 hours of work per fortnight.
The ideal candidate will have Australian hospital experience and be proficient in providing pharmaceutical services to patients and staff. They will cover a caseload, provide supervision and support to junior clinicians, and be responsible for the direct provision of pharmaceutical services.
The successful applicant will also contribute to the pharmacy team's oversight of the formulary and medication supply for the hospital, including controlled and highly specialised substances.
Key Responsibilities:- Work on rotation across various wards and clinics, providing a pharmacy service that includes resolving medication-related issues, supplying medications, and completing the Medication Reconciliation Process.
- Provide medicines information, counsel patients, and assist with the discharge process.
- Assist team members in delivering a pharmacy service and ensure appropriate drug utilisation through implementation of hospital policy and formulary guidelines/restrictions.
- Regularly monitor and assess medicine management to enhance medication safety.
- Australian hospital experience is mandatory.
