Job SummaryWe are seeking a skilled Dietitian to join our Community Home Care team in Melbourne South. As a Dietitian, you will play a vital role in providing nutrition and dietary services to clients with diverse needs and requirements.
Key Responsibilities- Conduct comprehensive assessments of clients' nutritional needs and develop personalized treatment plans.
- Provide expert advice on dietary requirements, meal planning, and nutrition education to clients and their families.
- Collaborate with multidisciplinary teams to ensure seamless care delivery and coordination.
- Communicate effectively with clients, families, and funding organizations to ensure clear understanding of care plans and progress.
- Member of the Dietitian Association of Australia (DAA) with Provisional or Full APD status.
- Valid drivers' license and reliable vehicle.
- Professional indemnity insurance.
- Valid NDIS Worker Screening Clearance.
- 2023 Flu Vaccine Certificate.
- COVID vaccine certificate.
