Job Title: Senior Patient Care Manager

Job Summary:

As a skilled registered nurse, you will lead the development, implementation and evaluation of patient care plans that cater to the unique needs of residents in our service.

To excel in this role, you will require sharp clinical assessment skills and the ability to take charge of providing high-quality patient services. You will form close relationships with each resident, becoming an integral part of their life and supporting them in achieving their individual goals.

You will work closely with medical and allied health professionals as well as residents' family members to advocate for residents' quality of life and ensure that care meets their individual needs. Additionally, you will provide guidance and support to our Personal Care Workers, utilizing your critical decision-making skills to lead the team effectively.

This role presents opportunities for growth and career advancement, including training to develop into various leadership roles such as service manager, deputy service manager, clinical nurse educator and other corporate positions.
