- Process intakes, develop care plans, and maintain accurate records
- Conduct comprehensive client assessments, focusing on Consumer Directed Care principles
- Coordinate the delivery of quality care, ensuring alignment with client needs, service plans, and available funding
- Deliver clinical in-home services, including care plan assessments, medication management, wound care, and chronic disease management
- Ensure accurate documentation and compliance with AHPRA regulations and legislation
- Current AHPRA registration as a Registered Nurse
- First Aid and CPR certification
- Australian National Police check
- Professional Indemnity Insurance
- Experience in assessing and planning care needs in a community setting
- Valid Driver's License and reliable vehicle
- Smart mobile phone (iPhone or Android)
- COVID-19 vaccinations
- Cert IV in Training and Assessment
- Exceptional communication and interpersonal skills
