The Role

This is a full-time, fixed-term position (12 months) with the potential for ongoing employment. As a Clinical Care Coordinator, you will play a critical role in coordinating and supporting the delivery of home-based healthcare to clients across Australia.



Key Responsibilities


  • Coordinating and managing acute nursing Hospital Care at Home programs for clients discharging early from hospital to home-based care
  • Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment
  • Implementing, monitoring, and modifying in-home care plans to meet client needs
  • Managing clinical case escalations and variations to expected clinical progress
  • Supporting clients to achieve their personal and clinical goals within their home environment
  • Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers, and funders
  • Helping to improve the health and wellbeing of hundreds of Australians


Requirements


  • Qualification as a Registered Nurse with current AHPRA registration
  • Experience with complex wound care and IV antibiotic management
  • Experience working in a multidisciplinary collaborative team
  • Excellent knowledge of hospital discharge planning
  • Ability to work in a dynamic environment with tight timeframes and competing priorities
  • Excellent clinical reasoning and problem-solving skills
  • Excellent verbal and written communications
  • Strong administration and time management skills, tech savvy
  • Passionate about health and committed to making a real difference in the community
