Care Partner Opportunity

We are seeking a dedicated and compassionate individual to join our Home Care team as a Care Partner. As a key member of our team, you will be responsible for providing home-based case management and care services to our clients in Hervey Bay.

Job Description:
  • Case Management: Develop and implement personalized care plans to meet the unique needs of our clients.
  • Care Coordination: Work closely with healthcare professionals, families, and other stakeholders to ensure seamless care delivery.
  • Client Support: Provide emotional support and companionship to our clients, promoting their dignity, independence, and overall well-being.
Requirements:
  • Qualifications: Certificate III in Individual Support (Ageing) or equivalent qualification.
  • Experience: At least 1 year of experience in a similar role, preferably in home care or aged care.
  • Skills: Excellent communication and interpersonal skills, ability to work independently, and strong problem-solving skills.
