We are seeking a highly skilled and experienced Registered Nurse to fill the role of Clinical Care Coordinator at our residential aged care centre in New Town, TAS 7008.
- This is a 6-month max term position with the potential for permanency.
- As the Clinical Care Coordinator, you will be responsible for coordinating the delivery of safe, effective, and individualised care to residents and their families.
- Provide clinical leadership in the coordination of nursing and care services.
- Promote and support a person-centred culture and values - respect and treat all people as unique individuals with needs, preferences, and a past history.
- Maintain accountability and responsibility for the clinical management of health and care outcomes of residents in the Centre.
- Ensure that skills and knowledge appropriate to delegate the task/authority have been considered when delegating health and care responsibilities to other professionals.
- Exercise critical thinking within an evidenced-based framework to ensure the delivery of high-quality health and care services which promote optimal health outcomes for residents.
- Deputise in the Care Manager's absence.
- Registered Nurse - Current unrestricted registration with Australian Health Practitioners Regulation Agency.
- Minimum four years of clinical experience as a Registered Nurse (preferably in Aged Care).
- Demonstrated knowledge in the care needs of the older person.
- Understanding of and commitment to continuous quality improvement.
- An understanding of and demonstrated experience with Aged Care Quality Standards and relevant Aged Care legislation.
- An understanding of and demonstrated experience with AN-ACC.
- Demonstrated excellent organisational and time management skills.
- High-level verbal and written communication skills.
- You must be a self-starter and able to work autonomously.
- Strong IT skills and experience in aged care clinical management using iCare.
