Job SummaryWe are seeking a highly skilled and compassionate Nurse to join our team as a Renal Care Specialist. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the success of our renal treatment centers.
Key Responsibilities:- Provide direct patient care to individuals with end-stage renal disease or chronic kidney disease, including administering medications, monitoring vital signs, and performing dialysis treatments.
- Collaborate with healthcare professionals to develop and implement individualized patient care plans, ensuring seamless transitions and optimal outcomes.
- Conduct thorough assessments, identify patient needs, and develop strategies to address complex medical and psychosocial issues.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices and policies.
- Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care and support.
- Associate Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N); three-year diploma from an accredited diploma program may be substituted for nursing degree.
- Current licensure as a Registered Nurse in the state of practice.
- Basic Life Support (BLS) certification.
- Experience in nephrology or dialysis nursing preferred, but not required.
