Senior Care Manager

Job Description:

This position involves managing a caseload of clients who require support with disabilities, child safety, and healthcare. The role requires providing expert advice and assistance to professionals regarding care practices for individuals with complex needs.

Key Responsibilities:
  1. Develop and implement care plans for assigned clients, ensuring their needs are met in accordance with relevant standards.
  2. Provide guidance and support to team members on best practice care management strategies.
  3. Represent the organization at tribunal hearings and other client-related issues.
  4. Collaborate with health professionals to develop treatment plans for clients with medical or mental health issues.
  5. Conduct regular supervision and training sessions for care staff, preparing reports as necessary.
  6. Participate in an after-hours contact roster, providing advice on managing critical incidents affecting clients.
  7. Ensure compliance with relevant quality frameworks and standards.
  8. Manage client finances appropriately.
  9. Engage with stakeholders to tailor service delivery to individual client needs.
Requirements:
  • Tertiary qualifications in Psychology, Nursing, Human Services, Social Work, Special Education, or related fields.
  • Relevant experience or postgraduate qualifications highly advantageous.
  • Demonstrated comprehensive knowledge of legislation, policy, and practice in disability care, child protection, aged care, or healthcare.
  • Current driver's license, COVID-19 vaccination, First Aid & CPR certification, Blue Card, NDIS Worker Screening Check, and LCS2 Background and History Check.
