Care Coordinators play a pivotal role in enhancing health outcomes for young people affected by mental health and/or alcohol and other substances. This position involves collaborating with clients, families, and communities to provide holistic and culturally appropriate care.

The successful candidate will work closely with the team to:

  • Encourage help-seeking behavior among young people and their families through early intervention services using evidence-based practice.
  • Support social recovery through education, training, and employment links.
  • Identify and respond to young people at risk of mental health crisis and related issues.

To be considered for this opportunity, applicants must possess:

  • Qualifications in healthcare, youth work, community work, counseling, leadership, or a related field.
  • Knowledge of evidence-based interventions and clinical practice.
  • Demonstrated experience in intake and screening activities.
  • Risk assessment and management skills.
  • Excellent communication and interpersonal skills.
  • Community engagement and advocacy skills.
  • Ability to prioritize and manage multiple tasks.
