We are seeking an exceptional individual to join our team as a Care Manager. In this role, you will play a vital part in shaping the lives of our residents and fostering a supportive team environment.

The successful candidate will be responsible for leading our care team with purpose, supporting their development, and making a meaningful difference to the lives of our residents every day.

  • You will work closely with the General Manager to manage and lead the delivery of person-centred care to residents, ensuring the highest standard of ongoing assessment, care planning, and evaluation and clinical governance.
  • Oversight of daily clinical operations, including coaching, mentoring, staff allocations, delegation, performance management, and education.
  • Reviewing clinical incidents, complaints, and changes, providing solutions to reduce any risks.
  • Leading compliance with AN-ACC assessments and evaluations.
  • Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.
  • Fostering a strong team environment that is collaborative, respectful, and inclusive.
  • Preparing and conducting clinical governance audits and evaluations.
  • Providing insights and innovative solutions for continuous clinical improvement.
  • Oversight of medication management in accordance with resident needs and legislative requirements.
  • Liaising with allied health, medical practitioners, and other services to provide optimal resident care.

What We Are Looking For:

  • Bachelor of Nursing with current APHRA registration.
  • Post graduate qualification in aged care highly regarded.
  • Strong leadership, communication, and interpersonal skills.
  • A person-centred care mindset that prioritises the needs and dignity of each resident.
  • A passion for nurturing a positive and productive team culture.
  • Ingrained empathy for our residents and a commitment to delivering high-quality care.
  • Proven track record in delivering quality clinical care within an aged care or acute setting.
  • Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
  • Proven and demonstrated experience in fostering a reporting culture balanced with effective investigative skills.
  • An understanding of Resident Rights, Aged Care Accreditation Standards, and Outcomes.
