About the Role

We are seeking an experienced and motivated Clinical Care Coordinator to join our team at Elizabeth Garden's Residential Aged Care Facility in Burwood.

Job Summary

The successful candidate will provide clinical leadership and work collaboratively with the Facility Manager and the care team, residents, and families to oversee and guide effective resident focused clinical outcomes.

Key Responsibilities
  • Undertake, or supervise others to undertake, clinical care for individual residents as required, including complex wound care, pain management, behaviour management, end of-life care and other clinical procedures as may be necessary.
  • Liaise with relevant Residential care staff, medical, allied health, hospital and other specialist health practitioners to ensure residents receive high level clinical care.
  • Focusing on clinical service delivery, compliance, and audit processes
  • Care planning in line with new standards

Requirements
  • Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA) administered through the Australian Health Practitioner Regulation Agency (AHPRA).
  • Recent experience in caring for high care and dementia residents.
  • Knowledge of wound care, palliative care, behaviour management, clinical assessments, care plans.

Please note that it is a mandatory requirement when working in the Aged Care sector to have a valid Police Clearance or National Disability Insurance Scheme (NDIS) Worker Screening Clearance, or potentially both, to undertake the inherent requirements of this role. All aged care workers must be vaccinated against Covid-19 (3 vaccines) and are encouraged to receive the Flu-Vaccine, all successful applicants will be required to provide proof of Covid-19 vaccination prior to commencement.
