As a key member of our care team, you will play a pivotal role in supporting residents to live their best lives.

**Key Responsibilities:**

  • Work collaboratively with the care team to develop and implement personalized care plans that meet each resident's unique needs.
  • Foster a culture of empathy and understanding, providing emotional support and promoting a sense of community among residents.
  • Coordinate care needs across the home, including medication management, nutrition, and hydration, ensuring seamless delivery of care services.
  • Identify and escalate concerns to the registered nurse or clinical care coordinator, ensuring timely interventions and effective resolutions.

**Requirements:**

  • Certificate IV in Ageing Support (including HLTHPS007 Administer & Monitor Medicines) or Diploma of Enrolled Nursing with APRHA Registration (endorsed).
  • Demonstrate high levels of commitment, integrity, and trustworthiness, adhering to organizational values and standards.
  • Exhibit excellent communication skills, both verbal and written, to effectively engage with residents, families, and interdisciplinary teams.
  • Show willingness to learn new skills and adapt to changing care practices, leveraging technology and resources to enhance service delivery.
  • Possess current National Police Check and NDIS Worker Screening Check, ensuring compliance with regulatory requirements.
  • Demonstrate working knowledge of WH&S and Infection Control protocols, prioritizing resident safety and well-being.
