CareGivers America, a leading provider of home care services, is seeking an RN/LPN to deliver high-quality patient care in the Sunbury area.

  • Schedule:Mon-Fri 6am-4:30p/Weekends 8:30a-8:30p
  • Area of Coverage: Sunbury

The ideal candidate will possess at least one year of experience in home care and have excellent communication skills. You Will Be Responsible For:

  • Conducting initial home visits to assess patient needs
  • Developing Plan of Care for each patient and continuously monitoring the need to revise or reevaluate based on patient's progress
  • Documenting daily clinical notes and progress updates on the patient's clinical record
  • Communicating with a patient's physician and other healthcare professionals to ensure delivery of appropriate patient care
  • Providing skilled nursing care as necessary
