Clinical Care Coordinator

We are seeking a highly skilled and experienced Clinical Care Coordinator to join our team at Infinite Aged Care. As a key member of our clinical leadership team, you will play a critical role in ensuring the highest quality of clinical care is delivered to our residents.
Key Responsibilities- Leadership and Clinical Expertise: Provide clinical leadership and expertise to our care teams, ensuring that all residents receive the best possible care.
- Care Planning and Coordination: Assist in the development and implementation of care plans, ensuring that all residents' needs are met and their care is coordinated effectively.
- Assessment and Documentation: Manage the assessment and documentation of residents' care needs, ensuring that all records are accurate and up-to-date.
- Quality Improvement: Identify areas for quality improvement and implement changes to ensure that our care services meet the highest standards.
- Team Management: Supervise and support our care teams, ensuring that they have the skills and resources they need to deliver high-quality care.
- Communication and Collaboration: Communicate effectively with residents, families, and other healthcare professionals to ensure that care is coordinated and residents' needs are met.
- Compliance and Accreditation: Ensure that our care services meet all relevant accreditation and regulatory requirements.
- Qualifications: Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
- Experience: Extensive experience in aged care, with a strong background in clinical leadership and care coordination.
- Skills: Excellent communication and interpersonal skills, with the ability to lead and motivate teams.
- Knowledge: Strong knowledge of aged care policies, procedures, and regulations, with the ability to apply this knowledge in a clinical setting.
