About the Role

We are seeking a highly skilled and compassionate Case Manager to join our team at Austin Health. As a Case Manager, you will play a vital role in providing high-quality clinical care to consumers, working collaboratively with our Consultant Psychiatrist and multidisciplinary team.

Key Responsibilities
  • Undertake assessments and case management of NEMSTS consumers, contributing to the implementation, monitoring, and evaluation of treatment objectives.
  • Deliver high-standard quality care for a clinical caseload, including consultation and liaison with family and organizations.
  • Provide assertive mobile outreach for consumers in their local community, ensuring timely and effective support.
  • Medication supervision to consumers in the community, promoting safe and effective treatment.
  • Collaborate with the Consultant Psychiatrist and Registrar to plan clinical interventions, ensuring implementation meets high standards.
  • Provide knowledge and skills as part of a multidisciplinary team, consulting with other mental health staff on specialist and community psychiatry matters.
About You

We are looking for someone with excellent written and verbal communication skills, able to work autonomously and as part of a team. You will bring energy and drive to your work, with a keen interest in developing your skillset.
