As a Discharge Planner at Healthscope, you will play a crucial role in ensuring optimal patient transitions from hospital to home. This position is designed for a dedicated professional who is passionate about enhancing patient care and experience.
Key Responsibilities:
- Facilitate effective discharge planning aligned with the hospital's strategic objectives and operational protocols.
- Ensure smooth patient flow and efficient bed utilization while collaborating with a dynamic healthcare team.
- Coordinate with nursing staff, allied health professionals, and medical practitioners to develop comprehensive discharge plans.
- Engage with external service providers to address ongoing care needs for patients post-discharge.
- Provide essential information and support to patients and their families regarding health and nursing care options.
- Conduct early assessments of inpatients with complex care requirements to promote timely discharges.
- Maintain accurate documentation of discharge plans accessible to all clinical staff.
Selection Criteria:
- A minimum of 2 to 4 years of experience as a Registered Nurse in an acute care setting.
- Strong interpersonal and time management skills, with the ability to thrive in a multidisciplinary team environment.
- Proficient in generating referrals to appropriate organizations.
- Capability to collaborate effectively with nursing unit managers and various departments.
