We are seeking a compassionate Registered Nurse to deliver high-quality care and empower clients to live safely and independently at home.

As a Community RN, you will play a key role in providing holistic, goal-directed care to clients across Home Care Package (HCP), CHSP, and other programs.

Key Responsibilities:

  • Deliver clinical care that meets the needs of diverse clients, including those with chronic conditions and complex care requirements.
  • Support health education and self-management through Telehealth and face-to-face visits.
  • Collaborate with Allied Health professionals to provide comprehensive, patient-centered care.
  • Promote healthy ageing and wellness through group programs and community engagement.

Requirements:

  • Current AHPRA registration as a Division 1 Registered Nurse.
  • Minimum 1 year of experience in community, aged care, or acute settings.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and healthcare teams.
  • Confidence to work autonomously with strong time management and organisational skills.
  • Advanced clinical care experience across areas like palliative care, chronic disease, wound and medication management.
