Salary

The successful candidate will receive an attractive salary of $90,000 - $120,000 per annum, commensurate with experience, depending on qualifications and years of service.

Job Description

In this rewarding role, you will be responsible for planning, assessing, implementing, and evaluating clinical care for patients, working collaboratively with families and the multidisciplinary team to achieve identified goals and health outcomes.

You will also contribute to the coordination of clinical care, providing direct and indirect clinical input, direction, and guidance to allied health team members, nursing staff, and non-clinical personnel.

Key Responsibilities:

  • Develop and implement individualized therapy programs tailored to meet the unique needs of each patient.
  • Conduct regular assessments and evaluations to monitor patient progress and adjust treatment plans as needed.
  • Collaborate with healthcare professionals to ensure seamless communication and coordination of care.
  • Provide education and support to patients and their carers/families to empower them in managing their health recovery, rehabilitation, and discharge plans.
  • Maintain accurate records and documentation of patient interactions, interventions, and outcomes.

Required Skills and Qualifications

To be successful in this role, you will need to possess:

  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • A minimum of 2-5 years post-graduate experience in generalist clinical settings, preferably with hospital-based rehabilitation experience.
  • Excellent time management, communication, and organisational skills, with the ability to work independently and as part of a multidisciplinary team.
  • Strong interpersonal and communication skills, with the ability to build rapport with patients, families, and healthcare professionals.
  • A commitment to ongoing learning and professional development, staying up-to-date with best practices and industry developments.
