About the Role:
We are seeking a highly skilled and compassionate Care Coordinator to join our team at Aged Care Resumes. As a Care Coordinator, you will play a vital role in supporting our clients to achieve their goals and live their best lives.
Key Responsibilities:
- Conduct initial assessments of prospective clients and provide recommendations on capacity
- Assess, monitor, review, and reassess the needs of clients within a holistic framework using a restorative and wellness approach
- Promote a harmonious team approach by acting as a mentor and supervisor to Home Care Workers to ensure delivery of community services
- Facilitate effective client pathway support through case discussions and case conferences, review of feedback, including incidents, hazards, and handovers, and monitoring of quality indicators
- Conduct annual reviews with clients, including revising their care plan, goals, services, and budgets to support our clients in living life to the fullest
Requirements:
- A clinical background as a Social Worker/EN/RN/Allied Health Practitioner/Case Manager - highly desirable
- Experience and expertise in the provision of services in Aged and Community services
- Experience and expertise in managing caseloads of clients with complex care needs
- Experience and expertise in working in a Clinical or Allied Health setting
- Experience in safety in the Aged Care or Health Sectors
- Experience in undertaking assessments, care planning, implementation, evaluation, and documentation of quality client care with a person-centred approach and in line with Consumer Directed Care principles
- The ability to communicate with staff, consumers, service providers, treating health professionals, and other community members to ensure the development of needs-based, consumer-focused service delivery
- Thorough understanding and ability to support compliance with Safety, Equal Opportunity, and other professional and legal requirements that affect day-to-day care and other services for clients
- Thrive in a team and appreciate the one-to-one interactions

The Essentials:
- A current Working with Children's check or willingness to obtain one
- Current and ongoing flu vaccinations
- Ability to pass a national police clearance
- Current First Aid and CPR certificates
