About the Role

This is a challenging yet rewarding opportunity to make a positive change in the lives of individuals with physical and mental disabilities.

Due to growth, we are looking for experienced support workers to provide assistance to our clients across the Perth region.

Working as part of a team, you will facilitate the delivery of high-quality services and achieve positive outcomes with service users in a supportive and proactive environment. You will be actively supporting their individual needs and creating a safe space.

Key Responsibilities:

  • Support participants to work towards achieving their individual goals.
  • Support daily living, promoting cooking, housekeeping, hygiene, shopping, medication administration, community access etc.
  • Support participants in building their confidence.
  • Liaise and collaborate with clients, their community and disability support agencies.
  • Monitor and review the participant's progress through the service and against the support plan.
  • Utilize positive behavior strategies.
  • Support clients to participate in age-appropriate activities and tasks, including employment, in-home disability support and community access.
  • Work in accordance with the medication administration policy, work health and safety procedures and client rights policies, including privacy and confidentiality.
  • Monitor the safety and well-being of individuals with disabilities and report any concerns to the Manager.
  • Ensure completion of appropriate documentation. As required, relevant information on disability support will be recorded in client notes and communication books.
  • Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
  • Work with clients, family/carers, relevant case managers and other professionals to support the client's individual goals.
  • Liaise extensively with other staff to ensure the overall smooth and consistent running of the disability services.
  • Report regularly to the Manager.

Skills & Experience:

  • Qualification preferably in community/disability/individual support/aged care/Mental Health and/or preferably with relevant experience.
  • Experience working with clients with challenging behaviors.
  • Well-developed verbal and written communication skills.
  • Current Driver Licence and reliable vehicle.
  • 100-point check ID.
  • Valid visa/working rights in Australia.
  • Has as below or willing to obtain prior to offer:
    • First aid/CPR.
    • Medication cert.
    • Manual handling cert.
    • Working with children check.
    • NDIS clearance check.
    • NDIS orientation module cert.
    • Covid-19 Vaccination evidence (1st, 2nd and booster doses).
    • Hand hygiene cert.
    • Covid-19 infection control cert.

How to Apply:

Pay: $30.33 - $35.00 per hour

  • 8 hour shift.

Supplementary Pay:

  • Penalty rates.
