Key Responsibilities- Supervise and coordinate clinical services for patients receiving home health care.
- Provide and direct nursing care to patients in their homes as prescribed by the physician.
- Coordinate determination of patient home health benefits, medical necessity, and insurance approvals.
- Assess patient needs continually and ensure care is individualized, appropriate, and meets eligibility criteria and physician orders.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to reliable transportation.
