Support Coordinator Position

We are seeking an experienced Support Coordinator to strengthen participants' ability to connect with and maintain supports outlined within their NDIS plan.

Key Responsibilities:
  • Support Coordination
  • Plan implementation
  • Progress and monitoring
  • Review and Reporting

The ideal candidate will have experience in NDIS Support Coordination or NDIS Planning, a minimum of 12 months experience in Support Coordination or NDIS Planning is required, along with a sound knowledge of the NDIS and local services.
