Job Description

As a seasoned nurse navigator, you will play a pivotal role in managing and coordinating ongoing care for vulnerable populations across all medical specialties. Your advanced practice skills will enable you to provide specialist nursing advice, consultation, leadership, and education to interdisciplinary teams.

  • Key Responsibilities:
  • Manage and coordinate ongoing care for frail, older consumers/patients across all specialties
  • Provide specialist nursing advice, consultation, leadership, and education to inter-professional team members
  • Practice in accordance with code of ethics, professional standards and legislation affecting nursing practice
  • Provide leadership and direction within the care services
Requirements

To succeed in this role, you must be able to fulfill the responsibilities in accordance with relevant policies and procedures. You will also need to maintain current knowledge and competence within your scope of practice, demonstrating commitment to health equity and meeting the intent of supporting Aboriginal and Torres Strait Islander people.
