Dedicated Care Professional

As a Care Coordinator, you will be responsible for ensuring quality and compliance governance are achieved through robust processes that meet internal and external auditing standards.
You will work closely with your Residential Services Manager to drive service excellence and resident wellbeing.
Key responsibilities include:
- Coordinating care provision within a large team to achieve high-quality outcomes
- Providing leadership, support, and mentorship to other team members
- Reviewing resident care and care plans to promote independence and quality of life
Requirements:
- Bachelor of Nursing or equivalent qualification
- Current AHPRA registration
- Experience coordinating care provision in a similar role
- Confidence to manage conflict, pressures, and competing priorities
- Ability to provide effective decision-making and clinical experience
