Supports Coordinator Opportunity

We are seeking a highly skilled Supports Coordinator to join our team. As a key member of our team, you will work closely with individuals with complex needs to provide coordination support and ensure they receive the right supports at the right time.

Your primary responsibility will be to assist participants in achieving their goals by connecting them with service providers, navigating the disability sector, and coordinating informal, mainstream, and funded supports.

You will have the opportunity to make a real difference in the lives of our clients and be part of a multidisciplinary team that values expertise and contribution.

Key Responsibilities:
  • Provide coordination support to individuals with complex needs
  • Connect participants with service providers and facilitate access to necessary supports
  • Navigate the disability sector to ensure seamless delivery of services
  • C Coordinate informal, mainstream, and funded supports to meet participant needs
Requirements:
  • Proven experience working with individuals with complex needs
  • Strong coordination and communication skills
  • Ability to build strong relationships with participants, families, and other stakeholders
  • Knowledge of the National Disability Insurance Scheme (NDIS) and its requirements
  • Ability to analyze data and present information to inform decision-making
