Our organization is seeking a dedicated professional to serve as a Care Coordinator. In this role, you will play a critical part in ensuring the highest standards of care for our residents.
- Monitor and evaluate resident care plans, providing expert advice on complex nursing needs.
- Continuously review and improve care delivery to maintain the highest quality of service.
- Regularly update care plans as necessary, ensuring accurate documentation in line with industry guidelines.
- Liaise with healthcare professionals, doctors, and specialist service providers to ensure comprehensive care.
- Manage resident conditions and progress, maintaining detailed records of ongoing care and interventions.
To succeed in this role, you will need:
- At least 5 years' experience in aged or healthcare, with a focus on leadership and interpersonal skills.
- Registered Nurse experience, with strong communication and numeracy skills.
- Ability to work collaboratively as part of a team, providing quality care and support to frail aged individuals.
- Sensitivity and understanding of the needs of the elderly in residential care settings.
