Social Rehabilitation Professional

This role involves working with consumers to develop daily activities and structures that improve their independent living skills by accessing appropriate leisure, vocational, and educational resources.
About the Role:- Utilizing social work expertise and discipline-specific skills to maximize individuals' recovery journeys.
- Working collaboratively with mental health and other service providers to facilitate positive rehabilitation and recovery opportunities through integrated care planning.
- Providing comprehensive bio-psychosocial assessments and developing individualized rehabilitation plans in collaboration with consumers and their families.
- Assisting consumers in achieving their goals and improving their quality of life.
- Developing and implementing comprehensive care/service plans to address consumer needs.
- Collaborating with multidisciplinary teams to ensure seamless care and support for consumers.
- Degree or equivalent qualification in Social Work which gives eligibility for full membership of the Australian Association of Social Workers.
- Extensive experience working within a mental health setting.
- Skills in undertaking comprehensive bio-psychosocial assessments, including risk assessments of consumers with high or complex needs.
