We are seeking a compassionate and skilled Home Care Support Worker to join our team in providing high-quality care and support to older persons living at home.
The ideal candidate will have strong interpersonal skills, ability to work independently and collaboratively as part of a multidisciplinary team, and demonstrate a commitment to person-centred care.
Key Responsibilities:
- Providing emotional and practical support to older persons living at home.
- Collaborating with the Client Services Manager and other healthcare professionals to develop and implement individualised care plans.
- Advocating for the rights and interests of older persons, promoting their dignity and autonomy.
- Participating in regular clinical reviews and meetings to ensure high-quality care and support is provided.
- Developing and maintaining relationships with clients, families, and carers to ensure ongoing support and care.
Requirements:
- Current registration with AHPRA as a Registered Nurse or Enrolled Nurse.
- Minimum 3 years' experience in aged care or home care.
- Strong communication and interpersonal skills.
- Ability to work autonomously and collaboratively as part of a multidisciplinary team.
- Commitment to person-centred care and continuous quality improvement.
