As a key member of our healthcare team, you will be responsible for providing high-quality patient care and working collaboratively with other healthcare professionals to achieve optimal health outcomes.
Key Responsibilities:
- Provide comprehensive primary care services to patients, including health promotion, disease prevention, and management of chronic conditions.
- Conduct thorough assessments, diagnose, and develop treatment plans for patients.
- Order diagnostic tests and interpret results, and prescribe medications as necessary.
- Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between healthcare settings.
- Document patient information and maintain accurate and up-to-date records.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
- Master's degree in Nursing or related field.
- Current registration as a Nurse Practitioner with the relevant professional organization.
- Minimum 2 years of experience in primary care or related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong analytical and problem-solving skills.
